How to Send a Case

To Submit Slide Consultations:

  1. Please submit a cover letter or fill out a requisition form and include any additional pertinent clinical information. Please also include the billing type requested and the patient’s insurance information if applicable.
  2. Place slides in plastic slide mailing containers and tape closed. Wrap slides in bubble wrap or use block mailers. Place the slides, blocks, and paperwork in a UPS or FedEx Envelope and ship to:

To Submit Tissue:

  1. Place labeled specimens with tightly closed lids in a biohazard bag along with a paper towel and seal the bag carefully. Place the folded requisition form and insurance information in the exterior bag pocket.
  2. Place the specimens in a second sealed leakproof bag.
  3. Place the specimens in a cardboard mailer with the label UN3373 and ship to:
