How to Send a Case – Slide Consultations:
To Submit Slide Consultations:
- Submit a cover letter or fill out a requisition form. Include any additional pertinent clinical information. Please also include your preferred billing type and the patient’s insurance information if applicable.
- Place slides in plastic slide-mailing containers and tape them closed. Wrap slides in bubble wrap or use a padded bubble envelope. Place the slides, blocks, and paperwork in a UPS or FedEx Envelope and ship to:

How to Send a Case – Wet Tissue:
1. Prepare the Order
- Place an EMA order or complete a Carbonless GSD Requisition Form.
- If the specimen is submitted by an outside clinic, include a copy of the patient’s insurance card.
2. Prepare the Specimen Package
Each case should be sealed in its individual biohazard bag.
Place the following documents in the side pocket of the biohazard bag in case the prefilled formalin bottle leaks:
- Patient requisition form
- Copy of the patient’s insurance card (outside clinics only)
Ensure that all formalin containers are:
- Tightly sealed
- Properly labeled
Place the sealed and labeled containers inside the biohazard bag. Securely press and seal the biohazard bag before placing it into the courier bag.
3. Complete the Manifest
- Prepare a shipping manifest and place it in the courier bag.
- GSD clinics should use the specimen tracking system and include a printed copy of the digital manifest.
- For outside clinics, reference the GSD Pathology Log
4. Send the Specimen
Send the package via courier to the LAB @ GSD WALNUT CREEK.
