3. Ship to Golden State Dermatopathology with FedEx
1. Place an EMA Order or Complete a GSD Requisition Form
including a copy of the patient’s insurance card if a non-GSD provider
2. Tightly Seal All Formalin Bottles
Place the patient requisition in the side pocket of the biohazard bag
